Well Child Checks Including Sports Physical 

  • Student will get sports physical form given to them at the event to take to their school

  • School Required Vaccines can be administered 

  • Consents will need to be signed

  • Insurance will be billed, but parent/guardian of student/athlete will not receive a bill

  • Please bring insurance cards

  • If you don’t have insurance, we will have Community Health Workers on hand to assist with signing up for OHP

  • If unable to attend the event, please call 541-677-7200 to get your child scheduled for their wellchild check including sports physical

NOTICE TO PATIENTS:
This practice serves all patients regardless of ability to pay.
Discounts for essential services are offered based on family size and income.
