Evergreen’s 2027 Insurance Network: What You Need to Know
Every year, insurance companies change their plans and networks. This page lists the health plans Evergreen Family Medicine is in network with for 2027 and highlights the changes that may affect you. If your plan is changing, your care team at Evergreen isn’t.
Changes for 2027

United Healthcare Medicare Advantage: Evergreen will not be in network with United Healthcare Medicare Advantage plans in 2027. United Healthcare commercial plans are still accepted.

Regence MedAdvantage PPO: In network through December 31, 2026 only.

Health Net Commercial (EPO, POS, PPO): In network through February 28, 2027.

Providence Health Plan: No longer accepted. Providence will no longer be providing coverage in 2027.

Humana: Humana Honor PPO and Humana National Employer HMO-Travel are not on our 2027 list.

Commercial plans: Yes, still accepted.
Medicare Advantage plans: No. Evergreen will not be in network with United Healthcare Medicare Advantage plans in 2027.
+ Does Evergreen accept VA Community Care?
No. Evergreen does not accept VA Community Care. If you also have another insurance plan listed on this page, please call us to confirm your coverage before your visit.
+ Do I have to leave Evergreen if my plan is changing?
No. Your care team is not changing. What’s changing is which insurance plans we’re in network with, so you may need to choose a new in-network plan during open enrollment.
+ When is open enrollment?
Medicare Annual Enrollment runs October 15 through December 7. Employer and marketplace plans follow their own timelines, so check with your employer or insurance company.
+ My insurance company’s website lists Evergreen as in network. Is that accurate?
Not always. Insurance provider directories aren’t always up to date. This page is the most current list of the plans we accept. If you’re unsure, give us a call.
